Delta State Schools of Midwifery Admission Form for the 2021/2022 Academic Session has been released. The deadline to apply is 20th March 2021 while the entrance exam will hold on 26th March 2021.

The General Public are hereby informed that the sales of admission forms into the Delta State Schools of Midwifery for the 2021/2022 academic session has commenced.

A. BASIC MIDWIFERY

Applications are invited from suitably qualified candidates for admission into the 3 years Midwifery Programme for the award of Registered Midwife (R.M) Certificate in the following Schools of Midwifery in Delta State:

• State School of Midwifery, Asaba;
• State School of Midwifey, Sapele; 

DURATION

• 3 years

ADMISSION REQUIREMENTS

• Applicants must be between 17 and 25 years of age and must have five credits at SSCE/WASC O/L. NECO in English Language, Mathematics, Biology, Physics and Chemistry at not more than two sittings

METHOD OF APPLICATION

Application Forms are obtainable in all the State Schools of Nursing and Midwifery. Application with enclosed receipt of payment and 2 passport photographs (colored with white background) must be routed through the Principal of the school of applicant’s choice.

B. COMMUNITY MIDWIFERY

The Common Entrance Examination Committee Asaba, announces its Entrance Examination into the following Schools of community Midwifery in Delta State, for admission of students into the 2-year Community Midwifery Programme for the award of Community Midwife Certificate.

• State School of Midwifery. Asaba
• State School of Midwifery, Sapele,

ENTRY QUALIFICATION:

• Five Credits at SSCE/WAEC/NECO at one or two sittings including English Language, Mathematics, Biology, Physics and Chemistry.
• Certificate and Birth certificate (not Affidavit). Age: 17-35.

ENTRANCE EXAMINATION FEE

• A NON REFUNDABLE Entrance Examination fee of N10,000.00 (Ten Thousand Naira) only. Payment should be made into the Ministry of Health, Department of Nursing and Midwifery account at Zenith Bank PLC.

Applicant with enclosed 2 passport photographs (coloured with white background) must be routed through the PRINCIPAL OF THE SCHOOL OF APPLICANT’S CHOICE. to: Secretary. Common Entrance Examination Committee, Asaba, Delta State, APPLICANTS WITHOUT THE NECESSARY ENTRY REQUIREMENT WILL BE DISQUALIFIED.
