University College Hospital Ibadan Community Health Officers Training Programme Admission Form 2025/2026 is now on sale. The form cost N20,000.

This is to inform the general public that the management of University College Hospital Ibadan has released the 2025 form for Community Health Officers Training Programme .

ENTRY REQUIREMENTS

A. EDUCATIONAL QUALIFICATIONS
Five (5) credit level passes in SSCE (WASSCE, NECO, or GCE O-Levels or NABTEB) at no more than two sittings of the same examination body which include English Language, Mathematics, Biology, Chemistry and Physics.

  • PROFESSIONAL QUALIFICATIONS AND EXPERIENCE FOR TWO (2) CALENDAR YEARS PROGRAMME
    In addition to qualifications in (a) above, candidates must possess any of the following professional qualification, and experience for eligibility to apply:
    Diploma in Community Health with not less than two years post-qualification experience.
    Community Health Extension Workers Certificate with five years’ experience.
    Applicant must be registered with the Community Health Practitioners Registration Board of Nigeria (CHPRBN) and possess current practicing license.
  • PROFESSIONAL QUALIFICATIONS AND EXPERIENCE FOR ONE (1) CALENDAR YEAR PROGRAMME
    Must have qualified as a Community Health Extension Worker.
    Possess first Degree in Community Health Science, Health Education, Public Health or HND in Community Health.
    Must possess current practicing license.

How To Apply for UCH Ibadan Community Health Officers Training Programme

The application form is available at the Community Health Officers’ Training Programme, UCH, Ibadan beginning from Monday, 24th March, 2025.
Interested candidates are to do the following:

  • Visit the UCH schools’ portal on https://portal.uch-ibadan.org.ng and click REMITA PAYMENT button to make payment of Twenty Thousand Naira (N20, 000.00) Only.
  • After successful payment, candidates are to sign up with his/her RRR, Email and Password used for the payment.
  • Login with your Email and Password to continue the online application
  • Go to the portal on https://portal.uch-ibadan.org.ng download the Applicants’ User Guide.
  • Print 2 Copies of your form and Photocard and bring them on the day of examination
