REQUIREMENTS: 

  • Original copies of Senior Secondary School Certificate (SSCE or its equivalent)
  • Indigene Letter 
  • Birth certificate/Declaration of Age
  • Secondary School leaving testimonial
  • Receipt for the purchase of form
