Federal University, Gusau notice on process of re-issuance of Identification Card 2023.


This is to bring to the attention of Staff and Students that the process of replacement of lost identity Card is as follows:
Staff or Student who lost his or her ID card is required to obtain a Court Affidavit after Police Report. Thereafter, he or she will generate Remitter and make payment of #5,000 only. He or she will then address an application letter to the Chief Security Officer, through his Head of Department and Dean, who are expected to certify his or her claim before the Chief Security Officer processes the issuance of the new ID card.
The application letter is to be accompanied with all the documents mentioned above.
