Federal University of Health Science Azare, FUHSA notice on physical screening exercise For 2025/2026 Academic Session.

I am pleased to invite the 2025 UTME/DE admission applicants for Physical Screening Exercise scheduled to hold as follows:

Physical screening will be conducted in the University as follows:

Tuesday 26th to Friday 29th August, 2025

Candidate’s date of Physical Screening is as indicated on the acknowledgement received after successful online registration

Candidates are advised to check their phones regularly for invitation to the Physical Screening, while disregarding any forms of communication except through the official channels, to avoid being scammed.

2. Candidates must bring originals of all their documents and one set of photocopies. The documents expected are:

·        Acknowledgement Form (printed from the FUHSA Post-UTME Portal)

·        UTME Result slip

·        ‘O’ Level Result (WAEC|NECO)

·        ‘A’ Level Result/ First Degree certificate (for Direct Entry only)

·        Scratch cards for ‘O’ level result(s) submitted

·        Evidence of Local Government Area of Origin

·        Birth Certificate or Sworn Affidavit of Age Declaration

3. Candidates are to check their registered phone numbers/emails for SMS/email of the invitation.
