UCH Ibadan SHIM Admission Form 2025/2026 is Out [UPDATED]

University College Hospital, Ibadan UCH School of Health Information Admission Form 2025/2026 Academic Session is Out.  The cut-off mark is 150.

Applications are invited from successful candidates who scored 150 and above in 2025 UTME (ND applicants) for admission into the School of Health Information Management of the University College Hospital, Ibadan for a Two- Year Programme leading to the award of the National Diploma (ND) and also a Two-Year Higher National Diploma (HND) Programme in Health Information Management. The course, which is non-residential, shall commence in September 2025.

ENTRY QUALIFICATION FOR ND AND HND PROGRAMME

FOR ND APPLICANTS Apart from the JAMB result, applicant must have obtained the minimum entry requirement of (5) credits at not more than two sittings in SSCE/GCE/WAEC/NECO from the same examination body in the following subjects:

• English Language
• Mathematics
• Physics
• Chemistry

FOR HND APPLICANTS: Applicant must present his/her ORIGINAL CERTIFICATES.

• O’ level Result (b) National Diploma or Professional Diploma in Health Information Management
• License Certificate (d) Letter of completion of Industrial Training (IT)

ENTRANCE EXAMINATION

The Computer Base Test (CBT) for ND will be in:

• English Language
• Mathematics
• Physics
• Chemistry
• Biology

The Computer Base Test (CBT) for HND will be in:

• Health Records Management
• Anatomy & Physiology
• Clinical Coding
• Biostatistics
• Computer Science
• Health Planning Management

How to Apply for UCH Ibadan SHIM Admission Form

Application form is available beginning from Monday 28th April, 2025 through the UCH SCHOOLS’ PORTAL:👉 https://portal.uch-ibadan.org.ng.

• Visit the UCH schools’ portal on https://portal.uch-ibadan.org.ng and click the REMITA PAYMENT button to make payment.
• After successful payment, candidates are to sign up with his/her RRR, Email and Password used for the payment.
• Login with your Email and Password to continue the online application
• Go to the portal on https://portal.uch-ibadan.org.ng download the Applicants User Guide.
• Follow instructions on the Applicants’ User Guide to input your data and submit your application online.
• Print 2 copies of your form and photocard and bring them to the examination center on the day of the examination.
• Make a payment of another Five Hundred Naira (N500.00) only into Health Information Managers Association of Nigeria, Account no: 0000207307 Union Bank, UCH Branch

The screening exercise for the ND programme and HND entrance examination will be communicated to the candidates via their e-mail address. Successful applicants will be invited for an oral interview at a later date through text messages and the School’s website.
