This is to inform all successful candidates admitted into COMMUNITY Midwifery programme at Kasimu Kofar Bai CONSKAT, Katsina and Nana Babajo CONSKAT, Malumfashi that ONLINE Registration will commence on Monday, 29th April, 2024 and ends on Sunday 12th May, 2024.

• DOCUMENTATION: All successful candidates MUST present to Kasimu Kofar Bai School of Nursing, Katsina, the ORIGINAL and 3 copies of the under-listed:

1. Admission Letter (To be printed online, COLOURED and on a cream CONQUEROR Paper)
   1. Biodata Printout
   2. Course Registration Form
   3. Payment Slip
   4. Completed Acceptance Form
   5. Completed ID Card Form
   6. Completed Undertaking Letter
   7. Completed Guarantor’s Form
   8. Medical Examination Form
   9. Six (6) Passport Photographs
   10. Acknowledgement Card
   11. SSCE Certificate or Statement of Result
   12. Secondary School Testimonial
   13. Primary Certificate
   14. Primary School Testimonial
   15. Indigene Letter
   16. Letter of Identification of Residence
   17. Hostel Fee Payment Slip (for those that will stay in hostel)
   18. Hostel Allocation Form (for those that will stay in hostel)
   19. Result Scratch Card(s)
   20. One (1) A4 Rim Paper

NOTE: Item b, c, d, e, f, g, h, i, p and q can be printed from the Registration Portal.

• Failure to register by Sunday, 12th May, 2024 means forfeiture of admission.
• Commencement of Lecture: Monday, 13th May, 2024.
