All 100Level and 200Level (direct Entry) students admitted in the 2022/2023 academic session as are requested to submit their two files in the Faculty administrative office on or before Wednesday 3rd 2023.

• Admission form
• Jamb results slip
• Course registration form
• Fresh students registration form
• Screening form
• Screening certificate 
• Identification form
• Birth certificate 
• Medical certificate of fitness 
• State of Origin Certificate 
• Admission letter pin
• School fees pin
• O level results 
• Matriculation Oath
